Initially, the Disturbed slot is played with 5 reels and 256 ways to win with a reel layout of 4x2x4x2x4. To land a winning combination, you need to see 3, 4 or 5 matching symbols appear from left to right, beginning from the leftmost reel), on adjacent reels and in any position. Only the highest win per way to win is awarded.

You can spin away as little as 20p per spin to as much as £100 a spin. With the maximum bet maybe limited to £10 or £5 per spin, you can change the bet by using the ‘$’ icon/button below the reels. The game is inspired by Harold Shipman who was an English serial killer doctor with an estimated 250 victims. Shipman targeted elderly patients with fatal doses of drugs of abnormal amounts of drugs prescribed (according to Wikipedia). He was nicknamed ‘Dr. Death’ and the ‘Angel of Death’.

The setting is an eerie and demented clinic which reinforces the game’s inspiration. You’ll instantly notice the 80s-style soundtrack which is surprisingly good and a little ‘Stranger Things’ in my opinion. It evokes feelings of dread and slasher movies.

On the reels, symbols include a range of Dr. Daniel Eath’s instruments which are both rusty and covered in blood. They include a stethoscope, scalpel and hand drill. Other symbols include a bedpan, medical kit, blood bag and biohazard waste bin. The higher-value symbols are represented by appropriately named patients Ms. Happ and Mr. Facelift and Dr. Daniel Eath himself. Dr. Daniel Eath is the best-paying symbol awarding 10 times your total bet for 5 across any way to win.

Disturbed, like NLC games such as Fire in the Hole 2 and Apocalypse, uses Enhancer Cells which brings special symbols into play. You have a Pill Bottle Wild that substitutes for all the paying symbols above. Other symbols include xWays, Mr.Split/xSplit, Disturber and Gravestones as well as Dr. Death ID Card and Executioner Scatters. With the functions of all special symbols highlighted in the Bonus Features section, below is a list of symbol payouts:

Disturbed Mobile

Mrs. Nudge 

When the Mrs. Nudge symbol lands on the middle reel, the Mrs. Nudge feature is triggered. You’ll see a woman inside a bin bag (obviously dead!). The symbol nudges to fully occupy the reel with every nudge increasing the win multiplier by 1. It’s a wild symbol so the win multiplier will apply to all wins involving the Mrs. Nudge wild reel.

Enhancer Cells

In the top and bottom positions of both reels 2 and 4, there are Enhancer Cells. On each spin, up to 4 Enhancer Cells can light up which will display several symbols. These award the following:

• xWays: A random position on the reels will display an xWays symbol which gives you more ways to win. It will also affect all symbols on an xSplit position.
• xSplit: A random symbol on the middle reel transforms into Mr. Split and can be chosen again. Acting as a wild, it splits all positions on the same row (except for itself) and adds 1 more symbol. If an xWays symbol is split, an extra xWays symbol is added.
• Wild: A random position transforms into a Pill Bottle Wild.
• Disturber: Every Disturber symbol triggers a respin which retriggers activated extras. Positions affected by xWays, Wild or split by the xSplit are not reset between Disturber.
• Gravestones: You may trigger the Angel of Death feature is you land 4 Gravestones (more details below).

If you use the Ramp Access feature, or xBet feature, the top left Enhancer Cell will be lit up on every spin. It will double your bet but a Dr. Death ID Card Scatter will also appear on the first reel. The RTP rate also rises slightly to 96.13% (for the 96.1% RTP version).

Pre-Ops Spins

3 or more Dr. Death ID Card Scatters will trigger the Pre-Op Spins feature. 3, 4 or 5 scatters will award 12, 15 or 20 free spins respectively.

With 1 Enhancer Cell always lighting up, activating a Disturber symbol will award 1 additional free spin. If you see the Executioner Scatter symbol on the middle reel, you will trigger the Annihilation Spins feature with 3 additional free spins.

Annihilation Spins

If you land 3 or more Dr. Death ID Card Scatters will all Enhancer Cells lit, the Annihilation Spins feature will be triggered. 3, 4 or 5 scatters will award 12, 15 or 20 free spins (respectively).

Unlike the Pre-Ops Spins feature, all Enhancer Cells will be lit throughout the feature. An additional free spin will be won when you activate a Disturber symbol.

Angel of Death 

The Angel of Death feature (one of Harold Shipman’s nicknames) is triggered when 4 Gravestones appear in the Enhancer Cells alongside a stacked Dr. Death symbol on the middle reel. This will ensure you win the max win of 54,391 times your total bet.

Nolimit Bonus

For non-UK players, there is an optional Nolimit Bonus feature (known as a Bonus Buy). There are 3 options ranging from 112 to 666 times your total bet. Furthermore, the RTP rate (for the 96.1% RTP version) also increases:

Theme-wise, it’s superb as you’d expect from an NLC production. I particularly love the 80s-style soundtrack which transforms into a heavy metal rage when you enter the free spins features.

 From a gameplay perspective, look out for the Mrs. Nudge symbol as this nudges to occupy the reel and increase the win multiplier with each nudge. However, it’s all about lighting up Enhancer Cells which brings various extras into play. If you manage to land 4 Gravestones plus a stacked Dr. Death symbol on reel 3, you’ll activate the Angel of Death feature and pocket the game’s jackpot or max win.

The 2 free spins features will see 1 or all 4 Enhancer Cells lighting up on each free spin. Naturally, bigger wins came in the Annihilation Spins feature but triggering it was very hard. With 54,391 times bet max wins, winning the top award in the Pre-Op Spins feature will occur every 57,000 spins on average. In the Annihilation Spins feature, it’s every 1,500 spins on average.

In summary, the Disturbed slot is gory, controversial, superbly designed and packs quite the punch regarding win potential. It’s an excellent game that has made it to our list of the best Halloween slots.
